Résumé

Cette étude a évalué l’effet clinique du vert d’indocyanine (ICG), dans la thérapie photodynamique antimicrobienne (aPDT), en une seule application, en tant qu’adjuvant au détartrage/surfaçage (D/S).

15 patients, non-fumeurs, atteints de parodontite chronique, dans la phase de maintenance parodontale, présentant au moins 2 sites dans 2 quadrants différents avec une profondeur de poches ≥ 5 mm, ont été inclus dans un essai clinique, randomisé, contrôlé, « split mouth » en double aveugle. Les sites ont été traités par allocation aléatoire comme suit : (A) D/S+ eau stérile + laser inactivé (site contrôle) ; (B) D/S+ICG activé par un laser diode d’une longueur d’onde de 810 nm (site expérimental). Les paramètres primaires étant : le niveau d’attache clinique relatif (RCAL) et la profondeur de poches (PD). Les paramètres secondaires étant : la position de la gencive marginale (PGM), la variation de la récession (varREC), l’indice de plaque-bouche totale (FMPS), l’indice de saignement (BOP) et l’échelle visuelle analogique (EVA).

L’évaluation des paramètres en préopératoire (T0) et la réévaluation à 3 mois (T1) et à 6 mois (T2) ont été réalisés par un second opérateur.

Le RCAL et La PD ont significativement diminué dans les 2 groupes à T1 sans différence significative entre les 2 groupes. Ils ont significativement diminué dans le groupe expérimental à T2, sans diminution significative dans le groupe contrôle. Mais la différence à T2 était significative en faveur du groupe expérimental.

L’aPDT utilisant l’ICG, en une seule application, en tant qu’adjuvant au traitement conventionnel durant la phase de maintenance parodontale, montre une amélioration des résultats cliniques dans l’échantillon étudié à comparer avec le groupe contrôle, dans le cadre des traitements non-invasifs de la parodontite chronique, chez les patients non-fumeurs à moyen terme (6 mois).
